AI Evaluation and Incident Infrastructure
We are deploying high-impact AI systems faster than we can measure their failure modes in the real world.
The Stakes
AI is becoming a control layer for finance, healthcare, logistics, and security. When a system fails at scale, harm scales. Without shared measurement and reporting, we cannot improve fast, regulate proportionately, or maintain trust in automation.
Headline evidence
- •NIST’s AI Risk Management Framework provides a common structure for managing AI risks across the lifecycle, but remains voluntary guidance. [1]
- •The MIT AI Incident Tracker catalogs more than 1,300 real-world reported incidents, indicating a sizable and growing harm surface. [2]
- •The EU AI Act rollout is staged over multiple years, implying a long compliance transition where operational evaluation tooling is still catching up. [3]
Neglectedness
Why it stays neglected
Incentives reward shipping, not measurement. Many incidents are reputationally costly to disclose. There is no universal incident schema. Monitoring is expensive, and external auditing markets are immature. Regulations are emerging, but operational standards and testbeds are still being built.
Tractability
Moderate to high. We can build test harnesses, continuous monitoring, and incident pipelines now. NIST is already piloting evaluation approaches. The hard part is aligning incentives, legal safe harbors for disclosure, and preventing metric gaming. AI can help by automating evaluation generation and anomaly detection.
Startup surfaces
- •Continuous evaluation platforms with domain-specific test suites and regression tracking
- •Incident reporting APIs and schemas integrated into production ML operations
- •Post-deployment monitoring tooling for drift, misuse, and emergent capabilities
- Benchmark-to-policy translation:compliance mapping for risk-based regulations
- •Independent auditing services with reproducible evaluation artifacts and attestations
